School-Based Health Center

Our School-Based Health Center (SBHC) located on school grounds is a primary care health center that serves the students of N.O. Charter Science & Mathematics High School. Health services are provided by the LSU Health Sciences Center Department of Pediatrics. The health center provides services to adolescents ages 10 and up. The health center accepts all patients with or without health insurance.

COVID-19

Weekly Testing

In partnership with the Louisiana Department of Health, NOLA Public Schools will offer COVID testing at no charge for all interested students. Testing will occur weekly on-site during school hours. A parent or guardian must register any minor under 18 years old.

Testing is through the Louisiana Department of Health at no cost. No insurance info is collected.

The Louisiana Department of Health is offering a monetary reward in exchange for participation in testing through this initiative. Students who participate in weekly testing 10 or more times between August and December can receive a $100 Prepaid card in December of 2022. Eligible participants will be emailed in December to confirm they would like to opt-into receiving funds.

A parent or guardian must register any minor under 18 years old. Registration is one time only, it does not need to be completed weekly. Please use the following link to sign-up for weekly testing. Register

The tests being used are PCR tests sent only to labs located within Louisiana to expedite results. Results should be expected on your COVIDSafe portal within 12-36 hours. You will also receive a notification via text and email (depending on what you provided) upon your results being ready.

COVID-19 Forms

New Orleans Charter Science and Mathematics High School take the health and safety of our students and their families very seriously. As such, in addition to steps to screen for the virus and prevent its spread on campus, we are adding a voluntary K-12 COVID-19 testing program for students. This program uses Abbott Laboratories BinaxNOW tests. We will only test with your consent. If you are willing to provide consent for us to administer this test on your child or yourself (if age 18 or older), please fill out this form.

Forms & Notices

All forms can be completed online but must be signed by hand. For those who may not have access to a computer or printer, all forms are available in person in the clinic. All forms require an original signature and should not be scanned or emailed.

Virtual Care involves the use of electronic communication to deliver clinical health care services between a practitioner and a client who are located in two different locations.
