Complete registration after placement by submitting required documents and forms.
Immunization records must be submitted with the state seal and expiration date. Please do not submit records from your health portal. To print your child's immunization record with a Louisiana state seal, please go to the Louisiana myIR website maintained by the Louisiana Department of Health. This site offers free copies of the state's immunization record and is the state's authorized record access system for you and your family's immunization records.
